Yellow fever (YF) is spread by the bite of an infected mosquito. is a vaccine preventable viral infection transmitted predominantly by certain species of day biting mosquitoes.

It is prevalent in tropical & sub-tropical regions of Africa and Central & South America. Outbreaks have caused up to a 50% mortality rate. The World Health Organisation (WHO) recommends vaccination if travelling to risk areas. Some countries require a certificate of proof of vaccination in order to enter. An International Certificate of Vaccination or Prophylaxis (ICVP) must be obtained from a registered yellow fever centre.

Signs & Symptoms

YF virus can cause an illness that results in:

  • High fever

  • Headache

  • Muscle pain

  • Nausea and vomiting

  • Sensitivity to light

  • Jaundice (yellowing of the skin and eyes)

  • Bleeding, with severe damage to the major organs.

The death rate is high in those who develop severe disease.

YYellow fever is a serious viral illness transmitted by infected mosquitoes, primarily in parts of sub-Saharan Africa, Central America, and South America. The disease is caused by the yellow fever virus, a flavivirus that affects the liver and other organs, and in severe cases, can lead to internal bleeding, organ failure, and death. The name “yellow” refers to the jaundice (yellowing of the skin and eyes) that can develop in more serious cases. While many infected individuals experience only mild flu-like symptoms or none at all, the disease can progress rapidly and unpredictably. There is no specific antiviral treatment for yellow fever, which makes prevention through vaccination especially important for those travelling to areas where the virus is present.

Yellow fever is spread through the bite of an infected mosquito, particularly the Aedes aegypti species, which is also responsible for transmitting diseases such as dengue and Zika virus. The mosquitoes typically breed in stagnant water and bite during daylight hours. The best method of prevention is through vaccination, which provides highly effective and long-lasting protection. In addition to vaccination, travellers are advised to take steps to avoid mosquito bites, such as using insect repellent, wearing long sleeves and trousers, and sleeping under mosquito nets when necessary. Many countries require proof of yellow fever vaccination for entry, particularly if arriving from a region where the disease is endemic.

The yellow fever vaccine provides long-term, and in most cases lifelong, protection after a single dose. The vaccine is known as Stamaril and is a live, attenuated virus vaccine administered as a one-time injection. It becomes effective around 10 days after administration. Although a single dose is now considered to confer lifelong immunity, some countries may still require a booster dose depending on their entry regulations or if the vaccine was received decades ago. The vaccination must be documented on an International Certificate of Vaccination or Prophylaxis (ICVP), which is required for entry into certain countries and must be issued by a registered yellow fever vaccination centre.
Yellow fever has a short incubation period, with symptoms usually appearing between three and six days after being bitten by an infected mosquito. In the early stages, it may resemble a mild flu with symptoms such as fever, chills, headache, back pain, muscle aches, nausea, and fatigue. Most people recover within a few days. However, in a smaller proportion of cases, the disease progresses to a more toxic phase with high fever, jaundice, abdominal pain, vomiting (sometimes with blood), and bleeding from the eyes, nose, mouth, or stomach. This severe form can lead to multi-organ failure and is often fatal without supportive care. Survivors of severe yellow fever may suffer from prolonged fatigue and liver complications.
The yellow fever vaccine is recommended or required for travel to many countries in sub-Saharan Africa and tropical South America, where the disease is endemic. It is also a legal requirement for entry into certain countries if you are travelling from, or transiting through, an area with a risk of yellow fever transmission. Countries such as Ghana, Brazil, Kenya, and Uganda are examples of destinations where vaccination may be either recommended for protection or required as a condition of entry. Failure to show proof of vaccination when requested may result in denied entry or mandatory quarantine. Travellers should check current requirements well in advance of their journey and ensure they are vaccinated at an approved yellow fever centre.
The yellow fever vaccine is generally safe, but as with all vaccines, some people may experience mild side effects. These can include soreness at the injection site, low-grade fever, headache, or muscle aches in the days following vaccination. These symptoms typically resolve quickly. In rare cases, more serious adverse reactions can occur, such as allergic reactions or severe neurological or organ-related side effects, particularly in individuals over the age of 60 or those with weakened immune systems. For this reason, a careful medical assessment is necessary before receiving the vaccine, and it may not be suitable for people with certain health conditions. Despite these risks, the vaccine remains the most effective way to protect against a dangerous and potentially fatal disease.
